- Marks Memo of 10th Class and Intermediate Examination
- Marks Memo of MBBS issued by the concerned University
- MBBS Final Degree Certificate (Provisional Certificate is permissible for those candidates only who passed the MBBS course in the year 2023 )
- Compulsory Rotatory Internship Certificate
- Permanent Medical Registration Certificate
- Study and Conduct Certificates from Intermediate to MBBS from the head of the Institution/College
- Social Status Certificate if applicable, issued by competent authority of Government of Telangana
- Relieving Order and NOC from the appointing authority (in case of In-Service Candidates)
- Bond as prescribed in Annexure – II & III
- Persons With Disability (PWD) certificate, where applicable
- Copies of Pan Card & Identity of the Candidate and the Sureties
