- MBBS degree certificate or provisional MBBS degree certificate.
- Marksheet: 3rd Professional Part 2 MBBS examination.
- Marksheet: 3rd Professional Part 1 MBBS examination.
- Marksheet: 2nd Professional MBBS examination.
- Marksheet: 1st Professional MBBS examination.
- Internship Completion Certificate (Internship must have been completed by 11.08.2023).
- Permanent or Provisional Registration certificate issued by Medical Council of India or a state Medical Council.
- Date of birth proof (Birth Certificate or Secondary or Higher Secondary examination admit card / marksheet issued by a recognized Board or Council stating date of birth)
- Photo ID proof (Passport / Voter ID card / AADHAR Card / PAN card / Driving licence)
- Certificate of SC / ST status in appropriate format.
- OBC Certificate (sub-caste must tally with central list) with non-creamy layer declaration in appropriate format.
- Disability Certificate (issued by one of the recognized disabilities certifying centres as notified by MCC) in appropriate format.
- EWS Certificate in appropriate format.
- Premature discontinuation bond on non-judicial stamp paper (not less than Rs. 10/-), duly filled in, signed and notarized.
- Government service bond on non-judicial stamp paper (not less than Rs. 10/-), duly filled in, signed and notarized or NOC from employer
- Proof of online fee payment.
- Fee payment declaration filled in and signed by candidate.
- Recent color passport size photo – 4 copies
